Procedures for DS forms
Procedures for Printing Accommodation Letters - R.A.L.F.
In order to use RALF to obtain your accommodation letters, you will need Adobe Reader installed on your computer. Adobe Reader is available through a FREE download at: http://www.adobe.com/products/acrobat/readstep2.html.
Below are the instructions to follow in order to obtain your accommodation letters:
1. Go to the DS website (www.lsu.edu/disability) and click on the Forms button located on the left side of the screen.
2. Click on the R.A.L.F. (Request Accommodation Letter Form) link.
3. You will automatically be linked to your PAWS account. Login to your PAWS account. Once logged in, the DS screen will appear.
4. Provide requested information in the boxes.
5. Check acceptance box at the bottom of the screen.
6. Click submit.
7. Next, click “Print Accommodation Letters” link and this will open an accommodation letter on your screen. Print one letter for each of your classes.
8. After you have printed your accommodation letters and if you received Note Taking as an accommodation click on the “Print Note Taking Packet” link to print your note taking information packet.
9. Last, provide each of your professors one of these accommodation letters and/or note taking packets during their scheduled office hours.
Procedures for Submitting Examination Request Forms
1. Go to www.lsu.edu/disability
2. Click on Forms on the left hand side of the webpage. Under Forms there will be a link
called Examination Request Form.
3. Click on Examination Request Form
4. Complete ENTIRE form (If anything is left blank, you will not be able to submit the form).
a. The Forms MUST be submitted at least 3 business days before the scheduled exam.
b. For Finals, you MUST submit your request at least five (5) business days prior to your exam.
5. Click on Submit
a. By submitting the form, it will send Disability Services and your professor an email.
**You are now registered for your exam**.
HOURS OF OPERATION: Monday – Friday 8:00am-4:30pm
Hours of Operation will be different for Finals Week during the Fall and Spring Semesters. Finals Week hours will be Monday – Friday 7:30am-9:00pm.and Saturday 7:30am-6:00pm
THINGS TO REMEMBER:
- When you schedule an exam remember that you have to be done with your exam no later than 4:30pm For example, if you have an exam that allows 1 hour to take and you get extended time as an accommodation, then you need to schedule your exam no later than 3:00pm. (EXCEPT during finals week)
- Show up to 112 Johnston Hall at the date and time of your exam. If you are more than 15 minutes late, DS will not administer the exam to you and you will have to reschedule your exam.
- You will be notified by two (2) emails once we have received and processed your Examination Request.
